This is how I did it:

...
<div type="book" osisID="Luke">
  <div type="introduction">
    <title>The Gospel of Luke</title>
    ...
  </div>
  <title>The Gospel of Luke</title>
  <chapter osisID="Luke.1">
  ...
  </chapter>
  ...
</div>

KDE bibletime does not display the introduction. I checked the
generated module, and the intro's are there. It thus appears to be a
limitation of bibletime.
