Thanks for the addition comment on notes applying to the entire 
transChange. Had not considered that.

Not sure we could enforce, from a schema standpoint, the use of <q> and 
related elements as milestones but we certainly could take note of your 
example and say in documetation that is how it should be handled, for 
the reasons you note.
