Having had no response, I've taken the plunge and edited the table myself.

These 6 CC licenses are now uppercase and have 4.0 appended.

Creative Commons: BY-NC-ND 4.0
Creative Commons: BY-NC-SA 4.0
Creative Commons: BY-NC 4.0
Creative Commons: BY-ND 4.0
Creative Commons: BY-SA 4.0
Creative Commons: BY 4.0

The 6 broken links have been removed.
The link below the list is now one that goes to the licenses page.

Notes:
1. We should still honour the form of words in existing modules, but new modules with a CC license should use one of the above exact form of words.
2. Any module requiring an update for separate reasons should have the DistributionLicense key updated if it was previously one of these CC licenses.
3. Creative Commons: CC0 remains unchanged.

Aside: I've also moved the Public Domain item to the top of the list.
