I've committed 2736/2737 to these files, cloning TEIHTMLHREF into TEIXHTML:

(I had to do a 2nd commit because I hadn't svn added the xhtml files
themselves.  Oops.)

Please check my work.  I did not make changes in

bindings/objc/src/SwordManager.h
bindings/objc/src/SwordManager.mm
bindings/objc/ObjCSword.xcodeproj/project.pbxproj

because there is inconsistent use of XHTML-related files in there.
