OK, now it's more than a curiosity.

First, I just did the conversion of Xiphos to use FMT_XHTML instead of
FMT_HTMLHREF.  No sweat, changed a few lines of code, rebuilt, worked
the first time.

However...

In the absence of a TEI XHTML capability, no TEI modules render
interesting content.  E.g. Geocoded and GeoAtlas have xref linkages, all
of which now come out as plain text, no linkage.

Is it time to clone teihtmlhref.cpp into teixhtml.cpp and update
src/mgr/markupfiltmgr.cpp to use it?
