[osis-core] additions to <hi>

No replies on this one. :-(
Interesting thing is that TEI has <hi> and <ref> that have almost the
same content models (varies under <ref> with some specialized term
containers).
Since we put <hi> in for convenience of encoders and I can easily
imagine a <reference> that has a <title> child, suggest we simply make
the two content models the same but leave them as separate elements.
I know, I know, Todd, that is not elegant, etc., but to some degree we
are bound to produce a spec our users will understand without blood
transfusions from one of the core team.
If there are any NO votes, let's hear them by late tomorrow shall we?
