[osis-core] additions to <hi>

Greetings,

After adding foreign, note, seg and reference to <hi>, the only 
remaining difference between <reference> and <hi> is that <reference> 
has <title>.

Is this a problem? Should we add <title> to <hi> and if so, how to we 
tell people when to use one or the other? Say <hi> is used only when 
unknown and otherwise use specific elements.
