Changed:

mileStoneStart and mileStoneEnd to set attributes to globalWithoutType

Type now set to mileStoneSE

Should both mileStoneStart and mileStoneEnd have a type? Isn't type on 
mileStoneStart sufficient? Since the two are linked together with start 
and end?

Deleted extension mechanism for type attribute on mileStoneStart and 
mileStoneEnd.

Shouldn't

    <xs:attribute name="who" type="xs:string"/>

        <xs:attribute name="level" type="xs:string"/>

be deleted from mileStoneEnd?

Added Chapter to milestoneSE attribute type.
