[osis-core] 3.2.2. Book Titles

Chris,

There are cases where a title is in more than one part but not a "sub".
"sub" indicates a primary/secondary idea.

It could be argued that this should/could be marked as a <lb/> within a
title.

I think I am convinced we should not have "continued" and use <lb/>.
