[osis-core] <chapter> in <speech> and not in <verse>

Todd,
We started off with <chapter> as a child of <p> (your request), broaden
by Chris/Troy to be a child of <lg>, and then "others."
If verses are broken to make new chapters, shouldn't chapter (as
milestones) be a child of verse as well?
