This is a feature of the parser, the index is always an index.docbook.

to changes this requires changing the DTD.

I was unable to get the section numbering to work correctly when using
the <set> tag, so my idea was to follow the kde.org recomendation not to
use it.  The best alternative seems to be to create several bibletime
document folders -> ~/docs/en/bibletime-handbook/index.docbook  ,  and
so on.
